Check-List for items to be included:
A minimum of 5 slides and up to 15 slides (total) of your artwork for
the BASIC webpage or up to 30 slides for the ADVANCED webpage.
One slide or photo of self/self-portrait (optional) not to exceed 5"x7"

Biography
Signed and Dated Contract
Your payment in the form of check, money order or credit card information for
the total amount of $250.00 or for the first installment of $70.00 (includes
12% fee) made payable to Woman Made.
A self addressed stamped envelope for the return of your slides.
